As Americans live longer with MCC’s, the likelihood of multiple drug therapies increase. Multiple medications may be necessary to maintain baseline functioning and to enhance quality of life. These facts become factors that impact the knowledge necessary in accurate medication reconciliation. In addition to being able to recognize medications by trade name, generic name, and drug class, it is also now necessary to have at least some minimum level of knowledge regarding treatment guidelines.
