Additionally, nursing staff should be checking the skin for redness, swelling, or breakdown with daily care, dressing, bathing, and toileting. All abnormal findings should be reported and documented according to your facility policy. Remember that prompt reporting and accurate documentation of abnormal skin findings help to protect you as well as the client. If you don’t report and document your findings, they may be reported by the caregiver on the next shift, and it may appear that the injuries occurred while the resident or client was under your direct care.
